Is Running Bad for Your “Bad Back”?
If you’ve been diagnosed with degenerative disc disease or chronic low back pain, you may have been told to avoid running because of its impact on the spine. However, emerging research suggests that the relationship between running and spine health may be more complicated—and more encouraging—than we once thought.
A 2026 analysis of the ASTEROID randomized controlled trial in the British Journal of Sports Medicine examined how a structured running program affected spinal discs. Researchers found that certain running characteristics were associated with favorable changes in MRI measurements of disc composition. Interestingly, people with greater levels of disc degeneration appeared to experience some of the most favorable changes.
This study unfortunately doesn’t prove running “cures” degenerative disc disease. Instead, it challenges the idea that a degenerative spine automatically needs to be protected from impact.
Your Spine Needs Movement
Intervertebral discs don’t have the same direct blood supply as many other tissues. They rely partly on movement and changes in pressure to help move fluids and nutrients in and out of the disc. Think of a disc like a sponge: loading and unloading creates pressure changes that help move fluid through the tissue. That means appropriate mechanical loading may be part of maintaining a healthy, adaptable spine.
What Did the Research Find?
Researchers found favorable associations with:
Running speed: Approximately 6.5–7.3 mph. More importantly, a moderate running speed is better for spine health than a slower shuffle or power walk.
Running volume: Approximately 17–28 miles over the 12-week study. Less than 1.5 miles a week and more than 2.5 miles per running session didn’t lead to significant results. This shows consistency in running is more important than being a weekend warrior.
Running surface: Predominantly grass. The results on surface loading were massively important and highlight that the more we can get onto natural surfaces instead of road and sidewalk running, the less “rebounding” forces our spine absorbs rather than distributing these forces through the ground.
Degenerative Discs Don’t Necessarily Mean “Stop Running”
Perhaps the most important takeaway is that an MRI doesn’t tell the whole story. Degenerative changes can exist alongside relatively normal function, and imaging findings need to be considered together with your symptoms, strength, mobility, running history, and response to activity. If you’ve stopped running because of back pain, the answer doesn’t have to be “never run again.” Instead, the goal may be to determine how much loading your body can tolerate right now and progressively build from there. That might mean starting with short run-walk intervals, gradually increasing training volume, addressing strength or mobility limitations, and paying attention to recovery.
